Fax the order form to (941) 739-2296 with your
credit card number, expiration date, and signature.

Send your order with a check to our post office
box. Add 6.5% for sales tax for shipping to a
Florida address. Also add $4 for shipping for
the first book, $1 for each book thereafter.
